Forms & Waivers

Forms & Waivers allow for the collection of additional information and the completion of agreements during First Step.

Overview

Forms & Waivers let you collect additional information and capture agreements—during First Step, during a store purchase, or on their own. Examples of information you can collect:

  • Student birthdate, allergies, hobbies
  • Parent interests

And agreements you can have signed:

  • Permission slips
  • Internet use policy
  • Photo policy
  • PTA waivers

To manage forms, navigate to Operations > Forms & Waivers.

The Visual Form Builder

Forms are built with the Visual Form Builder—a drag-and-drop editor. To open it:

  1. Go to Operations > Forms & Waivers.
  2. Click the form you want to edit to open its Responses page.
  3. Click the Visual Builder button.

The builder is a full-screen workspace with three areas:

  • Palette (left) — the form elements you can add.
  • Canvas (center) — your form. Click or drag an element from the palette to add it, and drag the handle on any element to reorder it.
  • Configuration panel (right) — slides in when you select an element so you can edit its label, mark it required, and set type-specific options.

Your changes save automatically—the header shows the current save status. Use Undo and Redo in the header to step through changes, and Preview to open the form in a new tab. Click the X to return to the Responses page.

Form Elements

Drag any of these elements from the palette onto your form:

Element Description
Display Text Rich text shown on the page (instructions, headings).
PDF Display an inline PDF document.
Short Text One-line text response.
Long Text Multi-line text response.
Checkbox Yes/no or true/false response.
Date Date picker.
Birth Date Capture the month and day—great for birthday boards.
Phone Number Phone number input.
File Upload Let the responder attach a document.
Signature Capture a digital signature online.
Custom Selector Choose from a custom list of options.
Grade Selector Choose from your school’s grades.
Teacher Selector Choose a teacher. Teachers are managed in Account Settings > First Step > Teachers. Not available on Student or Parent First Step forms.
Student Selector Choose a student from the current First Step session. Not available on Student or Parent First Step forms or shareable forms.
Parent Selector Choose a parent from the current First Step session. Not available on Student or Parent First Step forms or shareable forms.

Parent and Student First Step Forms

Student and parent forms are appended to the standard First Step shown on the signup page:

  • First Step Form: Parent
  • First Step Form: Student

These forms cannot be deleted—removing their elements has the same effect as removing the form. The teacher, student, and parent selectors are not available on these forms.

Requiring a Form on a Campaign

Some campaigns need to collect extra information or a signed waiver when a purchase is made:

  1. Open the campaign under Campaigns.
  2. Click the Edit button.
  3. Select the form in the Forms & Waivers section.
  4. Click Update to save the change.

Adding a Form to First Step

  • Open the First Step page where the form should appear.
  • Click Add Section, choose the Show Form option, and select the form.
  • Click Add First Step Section.

To limit the form to certain grades, open the section, click Edit Advanced Settings, choose the grades, and click Update First Step Section.

Sharing a Form

Forms can be used outside First Step and outside a campaign purchase. Open the form, click the Share tab, and download the QR code or copy the link. Both let people complete the form without an account or sign-in.

Exporting Responses

Information collected in a form can be exported by clicking Export Responses in the right sidebar:
